What happened:
On March 28, 2014 at around 9 am, a contract driver was travelling
to BVS station- to pick up the pipeline engineer, encountered with a big pot hole
on the road, drifted away from the road resulting in a roll over. The driver
escaped with minor injuries but the vehicle sustained damages.
Why it happened:
Poor Road Conditions and no
sign boards. High Speed of the vehicle.
No HSE Induction for the
driver and hence was unaware of the hazards.
Fatigue- The driver was
assigned for work for the previous 40 days without rest.
Lessons Learned:

Plan the journeys to Journey Management Plan.
ü
The road conditions shall be monitored regularly and repaired as
required.
ü
Proper sign boards will enable the driver to encounter the hazards
effectively.
ü Site HSE Induction shall
include details of road conditions which will enable new drivers to get familiarize
with it.
ü
Proper and sufficient rest to drivers will make them more alert
against hazards.
