What happened:
On 26th April 2014 at around 1030 hrs, a Kenworth trailer truck rolled
over in a road. The truck was loaded with chemicals to a drilling site. The
trailer while traversing through the sand track came across sand dunes which
made it impossible to travel further. The driver decided to reverse the vehicle
and in the process rolled over as the sand gave way in a sloped area. The
driver escaped unhurt with no apparent damage to the truck. Most of the materials
transported were damaged.
Why it happened:
The driver decided to reverse the vehicle even
after it was stuck in the loose sand. There was no assistance for the driver in
the vehicle.
The driver undertook the
journey through an unapproved track. No apparent communication between the
supervisor and driver for the route to be taken.
Lessons Learned:
ü Communication to drivers while
undertaking journey in desert roads shall be clear and precise.
ü The desert routes shall be
pre-approved prior to the journey.
ü A minimum of two people shall
be in the vehicles carrying heavy load during desert journeys for assistance.
