Vehicle Rollover

What happened:


On March 28, 2014 at around 9 am, a contract driver was travelling to BVS station- to pick up the pipeline engineer, encountered with a big pot hole on the road, drifted away from the road resulting in a roll over. The driver escaped with minor injuries but the vehicle sustained damages.

Why it happened:

Poor Road Conditions and no sign boards. High Speed of the vehicle.

No HSE Induction for the driver and hence was unaware of the hazards.

Fatigue- The driver was assigned for work for the previous 40 days without rest.

Lessons Learned:

ü   Plan the journeys to Journey Management Plan.

ü   The road conditions shall be monitored regularly and repaired as required.

ü   Proper sign boards will enable the driver to encounter the hazards effectively.

ü   Site HSE Induction shall include details of road conditions which will enable new drivers to get familiarize with it.
ü   Proper and sufficient rest to drivers will make them more alert against hazards.
         




What happened:


On 21st May 2014 at around 1050 hrs, a contractor crew was travelling in a Toyota Land Cruiser on the sand track for performing maintenance of the solar and transformer rectifier units. As they were maneuvering a sharp bend, the driver lost control of the vehicle resulting in a roll over. No injuries to the personnel reported but the vehicle sustained damage.

Why it happened:

The vehicle came down a steep slope before the 90o curve and did not anticipate the sharp bend ahead. As soon as he reached the bend, he applied brakes and immediately steered the vehicle to his right, the rear of the vehicle skid to the left and the roll over took place.

Driving on sand without engaging 4WD. Further the vehicle was over speeding.

The driver had poor safety record and the contractor management failed to take corrective actions.

Lessons Learned:

ü   Follow the posted speed limit –Speed in sand tracks shall be limited to a maximum of 80 km/hr.
ü   Driving on sand track requires engagement of 4WD.

ü   Driver performance shall be monitored and effective coaching to correct unsafe behavior be implemented


Vehicle Head on Collision

 

What happened:

On 15th May 2014 at around 0940 hrs, a private vehicle driven by an employee collided with a Contractor Vehicle at sand track. The employee was overtaking another vehicle but came face to face with the Contractor vehicle but could not steer away and both the vehicles rammed into each other. The employee sustained back injury and was admitted to hospital and the vehicles sustained significant damage.

Why it happened:

Improper Judgment: The employee in the private vehicle was overtaking without considering the upcoming vehicle.

Over Speeding – The private vehicle was driving at high speed.


Lessons Learned:

ü  Follow the posted speed limit –Speed in sand tracks shall be limited to a maximum of 80 km/hr.
ü  Extreme care shall be taken while overtaking. The driver shall judge the speed of the other vehicle and see the road ahead is clear.





Car Collision

What happened:


On 30th April 2014, a Rig vehicle (Toyota Land Cruiser) was proceeding to Rig Camp on a sand track. At the same time Operations crew vehicle (Double Cabin Pickup) crew was approaching towards an intersection on main track. The sand track was in low lying area with dunes, blocking clear view of approaching traffic at the intersection and the road warning sign was disoriented, due to winds. The rig vehicle was not fitted with desert flag. Both vehicles were driven at a speed of approximately 60 km/Hr. While Operations vehicle was in the middle of the intersection, the rig vehicle approached and collided with Operations crew vehicle.


All crew members and drivers escaped unhurt and the vehicles sustained minor damage.


Why it happened:

The driver entered the main track from a side track without stopping at the intersection.

Hazards associated with the route were not adequately identified and the warning sign was disoriented.
Rig vehicle was not fitted with desert flag and daily inspections failed to identify the same.


Lessons Learned:

ü Identify hazards associated with the route prior to commencing a journey in desert conditions.
ü When crossing intersections, ensure the road/tracks are clear.






Car Rollover

What happened:


On 26th April 2014 at around 1030 hrs, a Kenworth trailer truck rolled over in a road. The truck was loaded with chemicals to a drilling site. The trailer while traversing through the sand track came across sand dunes which made it impossible to travel further. The driver decided to reverse the vehicle and in the process rolled over as the sand gave way in a sloped area. The driver escaped unhurt with no apparent damage to the truck. Most of the materials transported were damaged.

Why it happened:

The driver decided to reverse the vehicle even after it was stuck in the loose sand. There was no assistance for the driver in the vehicle.
The driver undertook the journey through an unapproved track. No apparent communication between the supervisor and driver for the route to be taken.


Lessons Learned:

ü Communication to drivers while undertaking journey in desert roads shall be clear and precise.
ü The desert routes shall be pre-approved prior to the journey.
ü A minimum of two people shall be in the vehicles carrying heavy load during desert journeys for assistance.


Car Accident

CAR ACCIDENT
What happened:

On the morning of 17th April, 2014, Pipeline Engineer started driving car around 06:40am from accommodation to MOT having 3 other employees.

After driving approx. 500m at the speed of 76km/hr. he tried to change lane from right to left, while doing this he observed a passenger bus on his left side. Bus driver honked the horn; driver got panic and took a sharp turn to his right smashing four road side sign boards consecutively before stopping. All the four passengers including driver came out of the vehicle without injury, but damage to vehicle was irreparable.

Why it happened:

Unawareness of traffic rules and “Right-of-way”
Improper decision making and Lack of judgment
Absence of attention to the moving traffic


Lessons Learned:

ü  Always drive with full concentration and follow safe driving regulations and guidance.
ü  Identify road hazards and traffic flow on both sides
ü  Watch out for pedestrians especially near crossings and parked vehicles along road side.
